Stage C0: No Visible Signs, But Symptoms May Be Present

These early functional symptoms often go unrecognized or are attributed to other causes.
At this stage, lifestyle modifications such as increased physical activity, elevation of the legs, and compression stocking use can halt or significantly slow progression. Without intervention, however, venous insufficiency typically worsens over time.
Stage C1: Telangiectasias and Reticular Veins (Spider Veins)
At C1, small, dilated vessels appear beneath the skin’s surface. Spider veins (telangiectasias) are the fine, web-like red or purple vessels visible in the skin. Stage C2: Varicose Veins Become Visible
This is the stage most commonly associated with varicose veins in the public’s mind. The affected veins become visibly enlarged, ropy, and often protrude above the skin’s surface. They most commonly appear on the thighs, behind the knees, and along the calves. The underlying venous valves are clearly dysfunctional, allowing blood to pool and the vessel walls to expand.
Symptoms at this stage become more pronounced and can include aching, throbbing, heaviness, and cramping, often worsening at the end of the day or after prolonged activity. Restless leg sensations at night are also common.
Stage C3: Edema Without Skin Changes
At C3, chronic venous insufficiency causes persistent swelling (edema) of the ankles and lower legs. The swelling occurs because elevated venous pressure pushes fluid out of the vessel walls and into surrounding tissue. Unlike temporary swelling from a long day on your feet, C3 edema is persistent and may not fully resolve overnight with rest and elevation.
This stage signals that the venous system is under significant strain, and the risk of progression to skin changes is meaningful. A comprehensive evaluation including duplex ultrasound is recommended to map the full extent of venous reflux and guide treatment.
Stage C4: Skin Changes Develop
C4 is divided into two substages based on the type of skin changes present:
C4a – Pigmentation and Eczema
At C4a, increased venous pressure causes red blood cells to leak from vessel walls into surrounding tissue. As these cells break down, hemosiderin deposits create a characteristic brownish discoloration of the skin. Venous eczema, characterized by dry, itchy, inflamed skin, also commonly appears. These skin changes typically affect the lower legs and ankles, and they indicate that the underlying venous disease is advanced.
C4b – Lipodermatosclerosis and Atrophie Blanche
C4b involves more serious structural changes. Lipodermatosclerosis is a hardening and tightening of the skin and underlying fat tissue, often creating an inverted bottle-leg appearance. Atrophie blanche presents as pale, scarred patches of skin surrounded by dilated vessels. At this stage, the skin becomes fragile and highly susceptible to ulceration with even minor trauma.
Stage C5: Healed Venous Ulcer
A venous ulcer that has healed but left visible scarring defines C5. While healing represents progress, the underlying venous insufficiency has not resolved, meaning the risk of the ulcer reopening remains high without ongoing compression therapy and, ideally, treatment of the underlying reflux.
